pgh: fix upgrade URL — use 64-char registry key, not 'fleeky' placeholder
pgh app update parses pgh.upgrade as pgh://<64-hex-registry-key>/<name>; the schema-example 'fleeky' placeholder made 'pgh app update' fail with 'pgh:// URL must carry a 64-char hex registry key (got 6)'. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>
