Bump version to 0.4.0 — tree publish pass (Phase 0.5b)
DeployServer._scan now computes + persists a full tree hierarchy
alongside the file index. manifest.rootTreeHash exposes the root-level
tree hash for tree-aware consumers (pgh).
File-level sync unchanged — tree publish is additive and falls back
gracefully if walkTree fails.
