Add ignore=all to two problematic nested submodules
Ignoring nested submodules in: - repos/tier4/git-remote-punch-transport (has test/fixtures/test-repository with no URL) - repos/tier5/barely-node (has vendor/bare pointing to SSH GitHub URL) This prevents "No url found" and "Permission denied (publickey)" errors during recursive submodule updates on machines without GitHub SSH keys.
